Metastatic Prostate Cancer With PCWG3 Review
- 65 sites
- Bone scintigraphy, CT
- PCWG3 and RECIST 1.1
Challenge
Radiographic progression-free survival in metastatic prostate cancer depends on disciplined application of the PCWG3 2+2 rule, under which new bone lesions must be confirmed on a subsequent scan before progression is declared. Across 65 sites, this rule was being applied inconsistently, with the flare phenomenon and unconfirmed lesions variously counted or disregarded. Such inconsistency placed the accuracy of the trial's rPFS endpoint—and the integrity of event dating—squarely at risk.
Our Solution
Intrinsic Imaging embedded PCWG3 decision logic directly into the central reading workflow, so that no newly identified bone lesion could be scored as progression without confirmation on a later examination. Central readers assessed bone scintigraphy alongside CT under RECIST 1.1 for soft-tissue disease, with the confirmatory requirement enforced systematically rather than left to individual judgment. Every progression event was dated according to a uniform convention, producing a coherent and defensible rPFS dataset.
Results
- Central assessment of 2900+ bone scans
- Consistent rPFS event dating
- No outstanding progression-logic queries at database lock
